The Gift of Life until Death: Conversations for all Generations (3 part series)
Sa, 11/11, 1/20, 2/24, 8:45-10:30am, Mpls Campus Fellowship Hall; session on 1/20 will be in room 482/486. Cost: $30 for all three sessions. Register beginning 10/1: www.mtolivet.org, Julie Goodman 612-767-2208.

For most of us, it is a hard conversation that we would rather not have, but creating a plan with those you love for the end of life is one of the greatest gifts you can give them. Let us help to make it easier. Join us in this three-part series where you will learn how to have these conversations and create a plan that will allow loved ones to confidently carry out your wishes.
• Session1, Nov 11, will be facilitated by Annette Langdon, RN,BSN,MA Advanced Care Planning Liaison and ELCA Deacon at Fairview Health Services. Annette will help you start the tough conversations and keep them going. She also will share information about the process of Advanced Care Planning: Who will make sure my wishes are fulfilled? How do I choose a Health Care Agent? What types of resources do we need?
• At Session 2, Jan 20, Dr. Thomas J. Klemond, MD, Internal Medicine and Hospice/Palliative Care Specialist in Minnetonka, MN, will present the Health Care Directive document and help you to recognize its importance for yourself, your family, and our society. You also will receive all the information you need to fill out your own Health Care Directive, and be encouraged to have it completed by Session 3.

• For Session 3, Feb 24, Annette Langdon will return, along with a panel of professionals, to answer any final questions and help you put the finishing touches on your Health Care Directive document. In addition, a notary public will be present to notarize your final document.
Decide now to take the time to have this important conversation and complete your Health Care Directive. It truly is one of the greatest gifts you can give to those you love. We'll have a notary available to sign your Health Care Directive once it's complete.
If you are not able to make this Pastoral Care/Adult Education series, but are interested in completing a Health Care Directive, please contact Julie Goodman, 612.767.2208, julieg@mtolivet.org.
